1. Submit the following information to the delegate by the 30th day of the month following the end of each financial quarter: o A payroll summary report for all workers provided during the quarter, generated from the licensee’s bookkeeping software and showing, for all workers: hours worked, gross pay, net pay, tax deducted, superannuation amount, and any other deduction amount. o Documents relating to a sample of 5 employees for 1 month during the quarter including pay slips, timesheets, evidence of payment of wages, and VEVO check conducted (as relevant). The sample documents should relate to 5 different workers compared to the sample for the previous quarter, if possible. o Evidence of payment of superannuation contributions to all workers. o Australian Taxation Office Integrated Client Account summary showing all transactions for the quarter. o A copy of the statement for each business bank account held by the licensee, showing all transactions for the quarter. o Details of any subcontracting arrangements entered into during the quarter, including the person or entity name and ABN, and labour hire licence number. This includes any person or entity engaged by the licensee as a subcontractor and any person or entity who engages the licensee as a subcontractor. o Details of all users of labour hire services supplied by the licensee during the quarter, including the entity name and ABN. 2. The identified person may be involved in the licensee’s business only in the permitted manner. | ||
